Welcome to UpForm
UpForm is a form and popup builder for Shopify. Use it to create forms that collect inquiries, feedback, signups, bookings, and other customer information on your storefront.
This documentation follows the typical merchant journey: build a form, publish it, manage submissions and insights, connect integrations, and configure app settings.
Recommended learning path
- Follow Quick start to enable App Embed, create your first form, and publish it.
- Use Create a form for a complete walkthrough of the form builder.
- Review available fields in Builder elements.
- Choose a guide under Publish for the storefront placement you need.
- Submit a test entry, then confirm it in the Submission list.
Build a form
UpForm includes a visual form builder where you add fields, arrange layout, and configure how the form behaves.
You can start with a blank layout or a template, then configure fields, design, triggers, conditions, email content, and after-submission behavior. See Create a form, Builder elements, and Design.
Publish
You can show forms in different places on your storefront, including:
- A Shopify page
- A product page
- A collection page
- The homepage
- Popups or floating forms
- Other placements, depending on how you publish the form
Start with Publish on a page, or choose another placement under Publish in the sidebar. Removal guides are grouped there as well.
Submissions and insights
Submissions are stored in UpForm when Save submission data is enabled on the form. From there you can search, filter, export, and follow up with customers.
Integrations
After a submission, UpForm can send emails to store admins and customers, protect forms with CAPTCHA, and sync data with supported workflow providers.
- Configure email providers in Email configuration.
- Configure CAPTCHA credentials in CAPTCHA settings.
- Connect automation providers in Automation settings.
Provider-specific setup guides are grouped under Integrations in the sidebar.
Settings
App-wide settings control file storage, languages, error messages, and tracking. Start with General, then use the other guides under Settings as needed.
