Quick start

Get UpForm working on your store in three steps: turn on App Embed, create your first form, then publish it. These match the Get started checklist on the Dashboard.

Before you begin

  • Install UpForm from the Shopify App Store, or open it from your Shopify admin if it is already installed.
  • Use a theme that supports app embeds (Online Store 2.0 themes).
  • Email providers and CAPTCHA are optional for this quick start. You can add them after your first form is live.

Enable App Embed

Published forms only appear on the storefront when App Embed is on for your theme.

  1. In UpForm, open the Dashboard.
  2. In the Get started card, find Enable app embed.
  3. Click Activate App Embed (or Manage App Embed if you need to reopen the theme editor).
  4. In the Shopify theme editor, turn on the UpForm app embed.
  5. Click Save.

Return to UpForm. App Embed status on the dashboard should show Active.

If App Embed is off, publish actions may show Enable App Embed to display your forms. Turn it on before you continue.

Create your first form

  1. On the Dashboard, in Get started, click Create your first form, or go to FormsCreate new form.
  2. Choose a template (for example, a contact form) or a blank layout, then click Use template.
  3. In the form builder, open Form info:
    • Enter a Form title
    • Keep Active enabled
  4. Open Form elements and keep or adjust the fields you need (at least an email field if you plan to send notifications).
  5. Open General and enable Save submission data so submissions are stored and emails/workflows can run.
  6. Click Save.

Publish your form

  1. With the form open (or from Forms), click Publish form.
  2. Choose where the form should appear. For a simple first publish:
    • Set the form type to On-page in Design if it is not already
    • Under Select the page type, choose Page, Home page, Product, or Collection
    • Follow the on-screen options for an existing page or new page, then click Publish
  3. Or from the Dashboard Get started step Publish your forms, select a form and use Publish form.

You can also add a form as a theme section with Add form block / Add section when the modal offers those options.

Check that it works

  1. Open your online store (use the theme preview if the store is password-protected).
  2. Go to the page or location where you published the form.
  3. Confirm the form appears and submit a test entry.
  4. In UpForm, open Submissions and confirm the test submission is listed.

If the form does not show, confirm App Embed is Active, the form is Active, and the publish location matches the page you are viewing.
